1. REQUISITE DOCUMENTS FOR BUSINESS VISA

MANDATORY DOCUMENTS: Please show original document at the time of submission of application for verification.

1) Application Form Complete d visa application signe d by the applicant.

2).One recent passport size coloured photograph (white background).

3).Current Passport and Previous passport

Passport should have one blank page available and be valid for a period of 90 days beyond the date of return to Sri Lanka

Previous Passport’s if there any (If the previous passport lost, in the current passport have to mention that “previous passport reporte d lost”).

4).Two photocopies of passport bio-page and any amendments.

5).Letter from Sri Lankan Company explaining purpose of visit, professional status of applicant an d details regar ding the business relationship with the Italian Company.

6).Company Registration + Memorandum and Articles of Association (original and photocopy).

7).Letter of invitation from Italian Company (copy to be faxed - n. 2588622 - directly to the Italian Embassy)

8).Confirme d hotel reservation or proof of hospitality offere d by Italian Company.

9).Letter from Bank relating to the foreign exchange that will be issue d on receipt of visa (or) Current Cre dit car d statement - original an d photocopy.

10).Proof of adequate financial guarantees: salary slips for the last three months, personal and company bank account/s for the three months, income tax statements (personal and company) for last year

 11).Medical, travel and accident insurance policy as per travel itinerary - minimum cover Euros 30,000/-. (original and photocopy).

12).Confirme d airline ticket or reservation issue d by Airline/Travel Agency (original an d photocopy).  

13).For participation in International fairs/exhibitions in Italy , evi dence of full payment of stall (original an d photocopy). Documents relating to the exhibits being exporte d shoul d also be submitte d. 

14).Photocopies of visas issued in the last two years by Schengen countries, UK, Ireland, USA, Canada,Australia and New Zealand.

PLEASE NOTE:

ALL TYPE OF VISA REQUIRES INDIVIDUAL PASSPORTS ONLY.NO CHILDERN ON PARENTS PASSPORT.

The Embassy reserves the right to request for further documentation and to verify the documentation submitted. In this case the maximum period of 90 days fixed by the Italian law for the evaluation of this visa type will be suspended till verification is completed.

Applicant could always be requested to appear for interview.

2. REQUISITE DOCUMENTS FOR TOURIST VISA

MANDATORY DOCUMENTS: Please show original document at the time of submission of application for verification. (If one family is applying each applicant needs to submit the photocopies of all the documents as per the main applicant)

5).Covering letter from applicant explaining the purpose of visit an d travel plan.

6).Company Registration + Memoran dum an d Articles of Association (original an d photocopy). Applicable only for Sharehol ders of Sri Lankan Companies.

7).Letter re. employment an d leave approval to be furnishe d by Employer.

8).Confirme d hotel reservation or original sponsorship letter “Lettera di invito” (Photo copy of i dentification of Sponsor). (Please refer the link”- visa application forms “to download the form)

9).Letter from Bank relating to the foreign exchange that will be issue d on receipt of visa (or) Current Cre dit car d statement (or) Bank guarantee “Fi dejussione Bancaria” in favour of applicant prepare d by sponsor in Italy - original an d photocopy.

10).Proof of adequate financial guarantees: salary slips for the last three months, personal bank account/s for the last three months, income tax statements (last year).

11).Me dical, travel an d acci dent insurance policy as per travel itinerary - minimum cover Euros 30,000/-. (original an d photocopy).

12).Confirme d airline ticket or reservation issue d by Airline/Travel Agency (original an d photocopy).  

13).For minor children (under 18 years):

- visa application to be signe d by both parents or guar dian

.- birth certificate (Original and English translation) to be legalize d by Ministry of Foreign Affairs – Consular Division – Colombo .

.- letter of leave approval issue d by the School

.- a minor travelling with one Parent, should be personally accompanied by the other, in possession of a

valid passport, for the scheduled interview at the Embassy (or) in the event of the other Parent being abroad a letter of consent with the signature authenticated  by the closest Italian Embassy/Consulate of residence.   ...

.- if minor is travelling separately, both parents, in possession of vali d passports, have to be present with the minor for the sche dule d interview at the Embassy.

14).Marriage certificate (original + English translation) duly legalize d by the Ministry of Foreign Affairs (only applicable if husban d an d wife are travelling)

15).Parents whose chil dren are in Italy , have to submit the birth certificate of their chil d (original + English translation) duly legalize d by the Ministry of Foreign Affairs.

16).Photocopies of visas issued in the last two years by Schengen countries, UK, Ireland, USA, Canada,Australia and New Zealand  ...

3.REQUISITE DOCUMENTS FOR FAMILY REUNION VISA FOR JOINING SPOUSE RESIDENT IN ITALY /FOR CHILDREN/FOR DEPENDING PARENTS

(Wife or Husband (of age) of persons resident in Italy , not legally separated)

(parents of persons resident in Italy, without any other sons/daughters resident in the country of origin. parents of persons resident in Italy, more than 65 yrs of age, whose sons/daughters are resident in the country of origin and not in a position to sustain them (parents) due to personal health reasons of their own (sons/daughters)

MANDATORY DOCUMENTS:DOCUMENTS REQUIRED FOR THE PROCESS OF VERIFICATION

1) SPOUSE CATEGORY ONLY  Marriage certificate, original + English translation, duly legalized by the Ministry of Foreign Affairs thereafter to be translated into Italian.

2)MINOR/S CATEGORY ONLY (under 18 years)

1. Marriage certificate, original + English translation, duly legalized by the Ministry of Foreign Affairs thereafter to be translated into Italian

2. Birth certificate of minor, original + English translation, duly legalized by the Ministry of Foreign Affairs thereafter to be translated into Italian .                           

3. Visa application to be signed by both parents or guardian                                  

4. A minor travelling with one Parent, should be personally accompanied by the other, in possession of a valid passport for the submission of documents (or) in the event of the other Parent being abroad, a letter of consent with the signature authenticated  by the Comune, Italian (or) Sri Lanka Embassy/Consulate of  residence.                                       

5.  If both parents are living abroad a letter of consent (signature authenticated  by the Comune, Italian (or) Sri Lanka Embassy/Consulate of  residence) authorizing the guardian to accompany the minor when submitting documents.

3)DEPENDENT PARENT CATEGORY ONLY

1. Birth certificate of sponsor (son/daughter), original + English translation, duly legalized by the Ministry of Foreign Affairs thereafter to be translated into Italian.                            

2. Declaration by Area ‘Grama Sevaka’ relating to the family status of applicant (original + English translation, duly legalized by the Ministry of Foreign Affairs thereafter to be translated into Italian )                                

GENERAL DOCUMENTS FOR ALL TYPES OF FAMILY REUNION VISAS TO BE SUBMITTED ONLY AFTER THE ABOVE VERIFICATION PROCESS HAS BEEN COMPLETED

1) Application FormCompleted visa application signed by the applicant.                              

2) One recent passport size coloured photograph (white background).                               

3)Current Passport and Previous passport

Passport should have one blank page available and be valid for a period of 16 months beyond the date of submission.

Previous Passports if there any (If the previous passport lost, in the current passport have to mention that “previous passport reporte d lost” an d the police report- original + English translation, duly legalize d by the Ministry of Foreign Affairs thereafter to be translate d into Italian.)                          

4)Two photocopies of current and previous passport bio-page and amendment pages.                                

5)“Nulla Osta” (no objection certificate) in original, issued by the competent Prefettura in Italy                                 

6)Photocopy of the valid ‘Permesso di Soggiorno’ (Permit of Stay) of the sponsor                           

7)The relevant verified civil certificates ( Birth/Marriage / Death and Divorce Certificates)

  • ALL TYPE OF VISA REQUIRES INDIVIDUAL PASSPORTS ONLY.
  • All documents must be submitted in original. Documents such as Birth & Marriage Certificates, Nulla Osta.
  • The Embassy reserves the right to request for further documentation and to verify the documentation submitted. In this case the maximum period of 90 days fixed by the Italian law for the evaluation of this visa type will be suspended till verification is completed.

4. REQUISITE DOCUMENTS FOR EMPLOYMENT VISA

MANDATORY DOCUMENTS:

1).Application Form. Complete d visa application signe d by the applicant

2).One recent passport size coloure d photograph (white backgroun d).

3).Current Passport and Previous passport

Passport should have one blank page available and be valid for a period of 16 months beyond the date of submission. Previous passports to be submitted.

Previous Passport’s if there any (If the previous passport lost, an extract from the relevant Police Station, original + English translation, duly legalize d by the Ministry of Foreign Affairs.

4).Two photocopies of passport bio-page an d any amen dments.

5).“Lavoro Subor dinato” Nulla Osta (employment authorization – no objection) - photocopy.

6).Medical Report issued by the Embassy Doctor – Dr. A.M. Sebastiampillai - address : 166/12, Kirulapone Avenue, Colombo 05 – tel. 2514104 (Monday to Saturday : 8.30 a.m. to 1 p.m. – Monday, Tuesday, Thursday : 4  to 7 p.m. – Wednesday : 5 -7p.m.)

7).Birth certificate (original + English translation, duly legalized by the Ministry of Foreign Affairs.)

8).Declaration by Area ‘Grama Sevaka’ relating to the residence of applicant, original + English translation, duly legalized by the Ministry of Foreign Affairs.

9).Only for “Lavoro Stagionale” (Seasonal employment) category applicants, a flight reservation or airline ticket is required. 

SUPPORTING DOCUMENTS

5. REQUISITE DOCUMENTS FOR RE-ENTRY VISA MANDATORY DOCUMENTS:

MANDATORY DOCUMENTS: Please show original document at the time of submission of application for verification.

1).Application Form   Complete d visa application signe d by the applicant.

2).One recent passport size coloure d photograph (white backgroun d).

3).Current Passport and Previous passport

Passport should be valid for a period of 16 months and have one blank page available

Previous Passport’s if there any (If the previous passport lost, in the current passport have to mention that “previous passport reported lost” and the police report - original + English translation, duly legalized by the Ministry of Foreign Affairs thereafter to be translated into Italian.) ...

4).Two photocopies of passport bio-page an d any amen dments.

5).Covering letter (in English and Italian) from applicant detailing reasons for the re-entry application.

The following documents should be submitted according to each individual case:

1. Original postal receipts issue d by the “Poste Italiane” in favour of applicants requesting for a Permesso di Soggiorno “Foreigners Permit of Stay” or renewal of this document 2. If the “Poste Italiane” receipts/Permesso di Soggiorno was lost/stolen  in Sri Lanka , a Police report in original  translate d into English an d Italian. The original an d English translation of this report shoul d be duly atteste d by the Ministry of Foreign Affairs – Consular Division – Colombo .  

6).For minor children (under 18 years):

-visa application to be signe d by both parents or guar dian.

.- birth certificate to be legalize d by Ministry of Foreign Affairs - Consular Division - Colombo .

.- letter of leave approval issue d by the School.

  • Re-entry visa can be issued ONLY after authorization from the competent “Questura” (Police Headquarters) in Italy .

6. REQUISITE DOCUMENTS FOR RELIGIOUS PURPOSES

MANDATORY DOCUMENTS: Please show original document at the time of submission of application for verification.

1).Application FormCompleted visa application signed by the applicant.

2).One recent passport size coloured photograph (white background).

3).Current Passport and Previous passport

Passport should have one blank page available and be valid for a period of 90 days beyond the date of return to Sri Lanka .

Previous Passport’s if there any (If the previous passport lost, in the current passport have to mention that “previous passport reported lost”) .

4).Two photocopies of passport bio-page and any amendments.

5).Covering letter from applicant explaining the purpose of visit and travel plan.

6).Letter from the Sri Lankan Congregation and the Bishop of the Archdiocese explaining the religious purpose of the visit. 

7).Third Person Note from the Apostolic Nunciature – Colombo confirming the type of visa and the religious purpose of the visit.

8).Sponsorship letter (board, lodging and medical/travel/accident insurance) from the host Congregation in Italy attested by the Secretary of State – Vatican , furnishing details of the applicant and the religious purpose of the visit.

9).Medical, travel and accident insurance policy (if not provided by sponsor) as per travel itinerary - minimum cover Euros 30,000/- (original and photocopy).

10).Confirmed airline ticket or reservation issued by Airline/Travel Agency (original and photocopy).   
PLEASE NOTE:

  • Visa for religious purposes can be granted only to religious persons. Applicants who are not party a Religious order (or who do not belong to a religious order), but who intend to take part in pilgrimages or religious events should apply for a tourist visa (only for short visits, up to 90 days).
  • Applicants who intend (or are called upon) to attend novitiate courses, study or religious training should apply for a “study” visa.
  • For information concerning other religious denominations, please contact Visa Section directly.

7. REQUISITE DOCUMENTS FOR TRANSIT VISA

MANDATORY DOCUMENTS: Please show original document at the time of submission of application for verification.

6. A confirme d air ticket or reservation printout (original an d photocopy.
7. Evi dence of Financial Status during stay in transit a. Foreign Exchange en dorse d on passport. b. International Cre dit car d(s) . c. Others.
8. For Seamen joining ship a. Continuous Discharge Certificate (CDC). b. Letter from Shipping Agent. c. Letter from the Italian Shipping Agent, atteste d by the competent “Capitaneria di Porto” (Port Authority).

8. ON A PACKAGE TOUR (TOUR OPERATORS)

MANDATORY DOCUMENTS: Please show original document at the time of submission of application for verification.

3. Passport having vali dity of 90 days from date of expiry of visa with 1 blank page, issue d within the juris diction of the Embassy.
4. Covering letter from applicant detailing purpose of trip, duration of stay an d outlining the travel itinerary as well as the name of the tour operator organizing the trip.
5. Tour operator’s letter listing all the visa applicants in the family an d their passport number, with detaile d itinerary & receipt of services pai d an d assuring the return of all passengers.
6. Overseas me dical insurance with minimum coverage of Euro 30,000- with the inclusion of a repatriation flight if require d.
7. a. Foreign exchange en dorsement on the passport an d receipt or b. Cre dit car d copy along with statement of cre dit limit.
8. Proof of financial position: a. Letter from employer granting leave b. Salary slips c. Income Tax Statements d. Personal bank

9. Hotel Reservation
10. If sponsore d by Company then letter from employer confirming: a. The applicant’s position b. Date of employment c. Sanction of leave
11. For minor chil dren (application to be signe d by both parents) an approve d Leave Application from school is to be submitte d. Birth Certificate Legalize d by Ministry of Foreign Affairs. In case a minor is traveling unaccompanie d or with any one of the parents an “Affi davit” signe d by the parent leaving in Sri Lanka nee ds to be submitte d.

  • In case the above documents are found insufficient, the Embassy reserves the right to request any other proof of the applicants’ financial status/sources of revenue (properties/investments etc.). Furthermore applicants could be requested to appear for interview.
  • At the time of submission, photocopies of all documents to be returned need to be produced. The originals of the documents produced may be requested for totally at the Embassy’s discretion.

9. REQUISITE DOCUMENTS FOR SEAMAN VISA

MANDATORY DOCUMENTS: Please show original document at the time of submission of application for verification.

3. Passport having vali dity of 90 days from date of expiry of visa with 1 blank page, issue d within the juris diction of the Embassy.
4. Letter from the Shipping Company in Sri Lanka .
5. Fax from the Shipping agents in Italy . Fax from the Embassy of Greece, New Delhi
Details of fax:
a. Flagship of vessel b. Exact time of arrival an d exact time of departure of the vessel c. The fax shoul d be atteste d by the Harbour master port captain where the seaman is expecte d to join the vessel.
6. Copy of signe d crew agreement between the seaman an d the Shipping Company / employer.
7. Copy of the STCW Certificates.
8. Copy of the passport.
9. Copy of the CDC.
10. A confirme d air ticket or reservation printout (original an d photocopy).

10. REQUISITE DOCUMENTS FOR STUDY VISAS (GENERAL/RESEARCH)

MANDATORY DOCUMENTS:

3. Passport having vali dity of 90 days from date of expiry of visa with 1 blank page, issue d within the juris diction of the Embassy. (Two Photocopy of the passport bio page an d amen dment page.)
4.a. “Dichiarazione di Valore” (Declaration of Value) of the applicant’s e ducational /professional certificates duly legalize d by the Consular office of this Embassay. The University/Institute or School shoul d a dvise this Embassay directly in case it is not require d.
b. Documente d guarantees regar ding the stu dy course, professional formation or cultural activity un dertaken.
c. A dequate financial guarantees regar ding the means of sustenance. Not lower than the figures establishe d by the Internal Ministry as per Article 4, comma 3 of the T.U. 268/1998 or a Scholarship from the Italian institution in this connection. (refer attache d Table A) (click here)
d. Documentation regar ding the applicant’s economic an d professional status.
e. Acceptance of enrolment by the Italian School or University or institution, stating duration of the course.
f. Receipt of payment of tuition fees.
g. Overseas Me dical insurance for at least 30 days, to be complete d in Italy within the sai d 30 days with an un dertaking ( refer Declaration)
5. A d ditional requirements for STUDY VISA (Research)
a.If the stu dy activity is purely research oriente d an d does not involve any pai d professional activity/work, the inviting research institute shoul d give a declaration that there is no recourse to work (autonomous or subor dinate) an d that the scholarship involves a re-imbursement of expenses an d not a renumeration.
b. Kin dly note that un der the provisions of article 27 of the T.U. 286/1998 university professors an d researchers who will un dertake an aca demic task/job or a renumerative research activity with universities, institutes of instruction operating in Italy, will be issue d a work visa (subor dinate or autonomous as the case may be).
c. Whenever the formation activities involve practical intervention, an authorisation is require d for the qualification of con ducting professional stu dy activities by the “Ministero della Salute” (Ministry of Health) which entail the exercise of health activities. Failing this a no objection (Nulla Osta) issue d by the “Ministero della Salute” shoul d be presente d or a relevant declaration (Dichiarazione) from the Institution in Italy to be forwar de d to the “Ministero della Salute” with a copy to this Embassay.
6. “ The University / College / Institute you are applying to must issue a declaration that it is / is not recognize d by the competent Italian authorities.”

4. Covering letter from the Sri Lanka Sport Organization listing, all the names of visa applicants, detailing purpose of trip, duration of stay & outlining travel itinerary.
5. Covering an d detaile d letter (see above) from the Srilanka Sport Fe deration.
6. Covering letter from the Ministry of Youth an d Sport, listing all the names of the applicants an d detailing the duration of stay, the covering of the costs etc.
7. Invitation letter from the Italian Sport Organization, which is organizing the sport competition, listing all the names of visa applicants, duration of stay, covering of the costs etc., atteste d by CONI (Italian National Olympic Committee). Letter is to be sent to the Embassy an d to the SriLankan Organization.

Wherever there do not exist such Bo dies or the attestations for the intereste d categories cannot be issue d, these can be sustitute d by the professional curriculum supporte d by newspaper clippings, vi deo or au dio recor dings, printe d articles.
c. Inscription certificate of the firm in the register of enterprises.

5. A “Delega” must be signe d before this Embassay to nominate a person in Italy to apply for an d obtain the “Nulla Osta per familiare al seguito” on behalf of the principal applicant. The person to be nominate d for the “ delega” shoul d be someone in Italy e.g. the sponsor who can apply on behalf of the principal applicant to the competent “Questura” for the “Nulla Osta al familiare al seguito”.
6. A declaration ( dichiarazione per familiare al seguito) must be ma de in front of the Embassay with a copy of the i dentity document, requesting the presence of the spouse an d minor chil d/chil dren in Italy an d which attests that they shall be economically depen dent on the principal applicant. The requisite documentation will be provi de d by this Consular office.

  • “Nulla Osta per Familiare al Seguito” (No Objection certificate for accompanying spouse/minor children) in original, issued by the competent Questura in Italy . Kindly note that the “Nulla Osta” is valid only for a period up to 6 months from the date of issue. The Nulla Osta will have to be renewed on expiry.
  • Discrepancy, if any, in the names of the applicant/s on the “Nulla Osta” and the passport/s, should be corrected to correspond with the name on the passport.
  • Photocopy of the passport of the principal applicant who will work/study in Italy .
  • Airline reservation printout to be completed later with a confirmed airline ticket prior to retrieval of the visa.

7. For Accompanying Spouse in Italy

  • Original Marriage Certificate issued by the Registrar of Marriage Kaccheri clearly mentioning the registration number.

8. For Accompanying Parent in Italy

  • Birth Certificate of the child/children in original, issued by the Registrar of Births and Deaths, clearly mentioning the registration number.

9. Legalisation of all Certificates/Affidavits in Original.

  • Original certificates and affidavits to be duly validated by the Ministry Of Foregin Affairs. This Certificate should be translated into the Italian language by a recognized translator. The certificate and the translation should be legalised by the Consular section of this Embassay.

  • medical documentation issued by the country of residence attesting to applicant’s illness
  • declaration by a public or private Italian health care facility (this latter must be accredited by the national health service) indicating type of treatment, date initiated, estimated duration and costs
  • document from the Italian healthcare facility confirming the deposit of at least 30% of the estimated costs of the treatment requested or, alternatively, a specific administrative act passed by an Italian Region or specific authorisation issued by the Ministry of Health within the framework of humanitarian programmes.

6. Documentation proving the availability in Italy of resources sufficient for payment of remaining healthcare expenses, food and lodging outside the healthcare facility, and repatriation of the patient and eventual companion.
